The appointment request
This static preview does not send form entries to a website server or save them in browser storage. Your selected clinic, visit type and date are used in your browser to prepare a WhatsApp message. You choose whether to open WhatsApp and send it.
Do not enter medical records or detailed symptoms into the request form. Information you send directly to the practice through WhatsApp or email is handled through those services and the practice’s processes.
External services
Directions, Google reviews, WhatsApp and medical-source links open external services. Their privacy policies apply. The MAKAM AI anatomy viewer connects to its provider as the anatomy section approaches the screen. You can also open its standalone page.
Cookies and tracking
No analytics or advertising scripts have been added to this build. The anatomy provider and other linked services may have their own data practices. Hosting services may process standard request logs.
